ERHS Athletic Clearance Process

*We have a NEW clearance process via Final Forms. Here are the important notes:

1. You no longer need to see Mr. Kase or bring in/email any paperwork to get cleared.
2. You get cleared on your own by following the steps outlined in the email parents received on 8/9/24.

• Go to Final Forms.
• Login or follow “Never Logged In” button to gain access.
• Once logged in, you will be able to see your athlete’s physical expiration date (if one is in the system). 
• Parents must first select the desired sport (or ONE sport per season) for the athlete.
• Parents must complete their portion of the online forms.
• Please upload any new physical paperwork to your athlete’s Final Forms account.
•  Physicals turned in prior to this summer will not need to be uploaded.
•  Pay ASB ($40) and Athletic ($90) fees via via InTouch
•  Once parents have submitted the online forms, an email will be sent to the athlete’s school email.
• The athlete needs to click the link in the received email to complete the athlete’s online forms.
• Once this is completed, the athlete should appear green and is CLEARED to show up to practice.

** The AD will verify forms, eligibility, grades, etc., but appearing green will add the athlete to the sport’s roster and is a green light to show up to practice. **

*Baseline Concussion Screenings (BCS) are no longer required prior to participation in contact sports. Any athlete with an expired or nonexistent BCS in a contact sport will receive a concussion screening at practice in the first part of the season.

**If you previously emailed Mr. Kase physical paperwork for athletic clearance, please follow the steps above to get cleared.
